SCAN Health Plan announces an improved Provider Information Line at 1-877-270-7226.

Providers can obtain member eligibility verification 24 hours a day, seven days a week using our voice response system. You will have access to real-time eligibility data and have the option to listen to pending eligibility changes. Users of the Provider Information Line will be provided the following member eligibility data:

• Member name and ID number
• Member effective date
• Member termination date
• Primary Care Physician information
• Primary Care Physician effective date
• Medical group information
• Medical group effective date
• Benefit plan effective date
• Co-payment amounts for the following: PCP visits, specialist visits, DME, hospital, emergency room and lab
